OpenSSL produced an error: 256 upon adding new connection

trying to add a new road warrior connection (new hostname) and get error:
OpenSSL produced an error: 256
Cyphers chosen: SHA2 (512) and AES-GCM (256)
Last new connection was added in April without any problems. serial file is correct.
in the certs directory two files are produced: "hostname"req.pem and "hostname"key.pem
Any suggestions what’s wrong?
